Registration Procedures
To register for the 2023 Diabetes Educational Symposium, please fill out the registration form below.
Registration fees may be paid on line by credit card via PayPal, or by check.
If paying by check, the check must be made out to the Diabetes Coalition of Palm Beach County. Please indicate on the check memo line, Diabetes Symposium Registration Fee. The check should be mailed to:

Once the completed registration form and check are received, a confirmation receipt will be emailed to you.
