Registration Procedures

To register for the 2017 Diabetes Educational Symposium, please fill out the registration form below.

Registration fees may be paid by credit card via PayPal or by check.

If paying by check, the check must be made out to Genesis Community Health Inc. (the Diabetes Coalition’s fiscal agent). Please indicate on the check memo line, Diabetes Symposium Registration Fee. The check should be mailed to:
